The South Dakota Department of Health will be providing free flu shots to students on November 8th, parent consent is required. Please print and return the form on the following link: Printable Flu Shot Form by November 3rd if you want your student to receive a flu shot at school. If you are unable to print the form, please contact either Angela Smit (angela.smit@k12.sd.us) or Renee Johnson (renee.johnson@k12.sd.us ) and we will send one home with your student.
